What is Conservative Management and Why Must I Wait for Treatment?

We often have patients come to their initial appointment expecting to have their veins treated right then. However, there is a treatment process that must be followed, and with good reason.

When someone sees a vein specialist for leg vein issues, the treatment recommendation depends on the problem, and a correct diagnosis is critical. If the problem is simply spider veins in the legs, treatment can begin right away. If the person presents with signs and symptoms of venous reflux disease, most likley that person will need to undergo a conservative management period before treatment.

The first appointment with a vein specialist is the most important because this is when the initial diagnosis is made. Our surgeons meet with the patient to take a thorough history, conduct a physical exam and determine if further evaluation is required. Most patients then undergo a venous ultrasound test to identify the veins in the leg with underlying venous reflux disease. At The Vein Guys®, we conduct this test at our on-site IAC accredited vascular lab. If testing confirms venous reflux disease, patients must complete a conservative management period lasting 6 weeks to 3 months. During this time, patients are expected to exercise, elevate their legs, use NSAIDS for discomfort when needed, wear prescription compression stockings, and avoid prolonged periods of sitting or standing.

Not only is conservative management required by most insurance plans, it is also very important in the treatment process because it helps confirm the diagnosis of venous reflux disease. Conservative management should make the legs feel better for a while. If the legs don’t respond to conservative management, a different diagnosis should be considered.
